Click here to download a Summer Camp Information Packet. It contains important information about our camp including medical forms, permission forms, camp policies and frequently asked questions.

In compliance with regulations of the Mass Dept of Public Health (105 CMR 420) and local boards of health, we need to receive the following documents 2 weeks prior to the first day of camp in order for the player to participate:

1. A copy of the player's latest Immunization Records from your doctor
2. Completed Health Interview form
3. Completed Permission for Medical Treatment form
4. Completed Authorization List for picking up players

Please send or drop off all 4 documents for each player to:
